Accuracy
I certify that the information I provided is true and complete to the best of my knowledge. I understand that a material misrepresentation or omission may disqualify me or, if hired, may result in discipline up to termination, subject to applicable law.
Verification
I authorize Vidcom to contact the references I listed and to verify the employment, education, training, and credential information on this form. Vidcom will not request wage or salary history.
Screening and clearances
I understand that a conditional offer may depend on lawful, job-related screening or school clearance requirements. Any consumer report will use separate disclosures and authorization. This application is not authorization for a background report.
At-will employment
If hired, my employment with Vidcom will be at will. Either Vidcom or I may end the employment relationship at any time, with or without advance notice and with or without cause, subject to applicable law. This form, an interview, or a policy or handbook is not an employment contract. Only a written agreement signed by an authorized Vidcom officer can change at-will status.

Vidcom Filmworks, Inc. may obtain one or more consumer reports or investigative consumer reports about you for employment purposes.
These reports may include information about your criminal records, motor vehicle and driving records, identity verification and address history, employment history, education, and other public records.
Reports may be prepared by a consumer reporting agency. Vidcom currently uses Checkr, Inc., One Montgomery Street, Suite 2000, San Francisco, CA 94104, checkr.com, (844) 824-3247. You may request more information about the nature and scope of any report.
Reports may be obtained at any time after receipt of your authorization and, if you are hired, throughout your employment where permitted by law.
You will receive a copy of A Summary of Your Rights Under the Fair Credit Reporting Act with your authorization request. If information in a report may cause us not to hire you, you will receive a copy of the report, a copy of that summary of rights, and a reasonable opportunity to dispute or explain the information before any final decision.
If criminal history is considered, Vidcom will conduct the individualized assessment required by New York Correction Law Article 23-A. A copy of Article 23-A is available on request.

Background check authorization

I have read and understood the background check disclosure. I authorize Vidcom Filmworks, Inc. and its consumer reporting agency to obtain consumer reports and investigative consumer reports about me for employment purposes, including at any time during my employment where permitted by law.
I understand that I will provide my date of birth, Social Security number, and any other identifying information directly to the consumer reporting agency through its own secure portal, and that Vidcom does not collect that information on this form.
I understand I may request a copy of any report obtained about me.

Employee agreement
Four things, in plain language. This takes effect on your first paid day, not before.
1. Student information and confidentiality
Working a picture day means handling student names, rosters, grades, photographs, and images. New York Education Law 2-d and the federal Family Educational Rights and Privacy Act treat that as protected student data, and our contracts with school districts require us to hold every person on our crew to the same standard.
I agree that I will use student and customer information only to do my assigned Vidcom work. I will not copy, keep, post, share, text, email, or upload student images or information outside Vidcom systems. I will not photograph students for my own portfolio or social media. I will report any suspected loss or exposure of student data to Vidcom immediately. This obligation continues after my employment ends.
Software, systems, and equipment. Vidcom's software, platforms, accounts, and equipment configurations are confidential business information. I agree that I will use Vidcom logins, accounts, licenses, apps, and equipment only for assigned Vidcom work and only under my own credentials. I will not share, transfer, or reuse Vidcom credentials, license keys, or account access; will not install Vidcom-licensed software on personal devices without written permission; will not connect personal storage or unauthorized devices to Vidcom systems; and will not disclose which platforms, workflows, camera or lighting configurations, or vendor tools Vidcom uses. I will return or delete all Vidcom software, files, and access when my employment ends.
I also agree to keep Vidcom's other business information confidential, including pricing, school contracts and contacts, order data, and vendor terms.
2. Not soliciting Vidcom accounts
For twelve months after my employment ends, I will not solicit, service, or attempt to take away photography business from any school, district, league, or organization that was a Vidcom account during my employment and that I personally worked on or learned confidential information about.
This does not stop me from working as a photographer, taking any other photography job, or working for a competitor. It applies only to Vidcom's own accounts.
3. Photographs and work product
All photographs, video, image files, edits, and other work I create in the course of my Vidcom employment are works made for hire and are owned by Vidcom Filmworks, Inc. To the extent any of it is not automatically a work made for hire, I assign it to Vidcom. I will deliver all files and memory cards to Vidcom and will not retain personal copies of student or customer images.
4. Equipment
Vidcom equipment issued to me stays Vidcom property. I will use it for Vidcom work, take reasonable care of it, report damage or loss promptly, and return everything when my employment ends or when asked.
This agreement does not change the at-will nature of employment, and nothing in it prevents me from discussing wages or working conditions, or from reporting suspected unlawful conduct to a government agency.
